The report revealed that mistakes by VA employees during the processing of claims for veterans rated as 100 percent disabled have led to roughly $100 million in incorrect payments. According to the VA watchdog, the errors stem from claims processors failing to follow established policies and procedures. The report estimates that between May 2022 and April 2023, approximately 75 percent of claims granted or denied for veterans with a 100 percent disability rating were improperly handled. The mistakes have caused veterans to receive incorrect compensation, prolonging the time it takes to resolve their claims and, in many cases, leaving them without the full benefits they are owed.

The report noted that many claims processors hired under the PACT Act, which expanded disability benefits for toxic exposure, were not adequately trained, leading to increased mistakes. In response, the VA has committed to improving training and oversight.
